CLAY COUNTY, Fla. — An 84-year-old man is dead and a 23-year-old woman is in serious condition after a crash on State Road 16 in Clay County Monday night.

According to the Florida Highway Patrol, the man was driving a red pickup truck heading west on SR-16 just west of the Shands Bridge. At the same time, the woman was driving a gray Kia heading east on SR-16.

Troopers said the pickup truck crossed the yellow centerlines and crashed into the Kia heading in the opposite direction.

Just after the crash, a third vehicle crashed into the debris left behind from the initial crash.

First responders arrived on the scene to render aid to the drivers.

The 84-year-old man was pronounced dead at the scene, FHP said.

The 23-year-old woman was taken in serious condition to the HCA Florida Orange Park Hospital for further treatment.

The driver of the third vehicle was not injured.

Right now, it's unclear why the pickup driver veered into oncoming lanes.
